Staff Attestation – COVID-19 Vaccination

To support a safe return to in-person learning, the Ministry of Education has implemented a mandatory vaccination disclosure policy for school boards for the 2021-2022 school year.

All permanent and casual employees of the Consortium de transport scolaire d’Ottawa bus contractors, must submit a COVID-19 Vaccination Attestation by completing this form.

IMPORTANT: You will be asked to provide proof of vaccination at a later date.

Individuals who do not intend to be vaccinated, without a documented medical reason, will be required to participate in an educational session about the benefits of COVID-19 vaccination.

Regular rapid antigen testing will be required for all staff who are not fully vaccinated against COVID-19.

In accordance with the Municipal Freedom of Information and Protection of Privacy Act, personal information is being collected under the authority of the Ministry of Education and will be used for the management of COVID-19 in schools. School boards are required to provide aggregated and depersonalized statistical information to the Ministry of Education and publicly disclose board-level immunization status data on a regular basis.
